Medicaid FFS cells are parsed from official state files for Alaska, Arizona, California, District of Columbia, Florida, Illinois, Indiana, Kentucky, Louisiana, Michigan, Missouri, Mississippi, Montana, North Dakota, Nebraska, Nevada, New York, Ohio, Oklahoma, South Carolina, Texas, Virginia, Washington, West Virginia. Medicare and Medicaid fee-for-service amounts come from those published schedules. UnitedHealthcare medians come from the published Transparency in Coverage in-network file when we have parsed it.
